Health Home Care Management
Health Home Targeted Care Management is a service model where all of the professionals involved in your care communicate with one another so that all your needs (medical, behavioral health and social service) are addressed in a comprehensive manner. At HVCS we have thirty years of experience providing quality compassionate care and managing service coordination for chronically ill individuals throughout the Hudson Valley.
A Medicaid Health Home is an optional service created under the Affordable Care Act of 2010 for individuals with Medicaid who have specific chronic conditions. A Health Home is not a physical space. A Health Home is a care management service model.
Our Care Managers will oversee and coordinate access to all of the services you require while ensuring you receive everything necessary to stay healthy.
These services include:
- Scheduling and remembering appointments
- Communicating with doctors
- Getting answers to questions about health care
- Avoiding unnecessary emergency room visits and hospitalizations
- Accessing housing, transportation, and food programs
- Maintaining health insurance
- Managing medications
- Comprehensive transitional care from hospitals or rehabilitation facilities
- Coordinating services with all providers involved
To be eligible, you must have Medicaid and:
- Chronic mental illness
- HIV/AIDS
Or TWO chronic conditions:
- Cancer
- Chronic Pain
- Heart Disease/Cardiovascular Disease
- Hypertension
- Diabetes
- Asthma
- COPD
- Obesity
- Chronic Renal Failure
- Addiction Disorder
